Form Completion and Letters of Support
What kind of help can OTA provide?
OT Assessed can assist with completing forms that require input from allied health professionals.
Common examples include the NDIS Home and Living Supporting Evidence Form, and government housing forms such as the Independent Living Skills Assessment or Medical Assessment.
If you need help completing a form, please attach it to your referral. One of our OTs will review it and advise whether it is suitable for us to complete.
Letters of recommendation
We can also provide formal letters of recommendation to support:
Urgent housing requirements (including urgent housing requests, transfer requests, or modification requests)
Assistive equipment
Please note, we do not conduct full assessments for home modifications or assistive technology. We only provide formal letters of recommendation to support approval or to serve as additional supporting evidence alongside existing assessments. If you have a specific request for a letter of recommendation, please include the details in your referral.
