What OT Assessed can do for you
Functional Capacity Assessment (FCA)
An FCA is a comprehensive assessment that assesses a Participant’s ability to perform daily activities across different environments, identifies support needs, and informs NDIS funding decisions.
Short-Term Accommodation (STA) Assessment
Short-Term Accommodation is funding for respite support and accommodation for a short time away from your usual home. This assessment involves a standard FCA with an extended section addressing home and living needs to justify the requirement for respite support.
Independent Living Options (ILO) Assessment
Independent Living Option (ILO) is a funding package that supports Participants (18+) to live independently at home. It is intended for those who require support for at least six hours per day. This assessment includes a standard FCA with an extended section addressing home and living needs to justify the requirement for ILO support.
Supported Independent Living (SIL) Assessment
Supported Independent Living (SIL) is a funding stream for Participants with high support needs who require significant assistance throughout the day. Typically, eligibility for SIL requires a need for 24-hour support. This assessment includes a standard FCA with an extended section addressing home and living needs to determine a safe support ratio and clearly justify how the 24 hours of support should be used.
Completion of Forms
For NDIS, housing, or other forms that need to be completed by allied health professionals. This may include government housing forms such as the Independent Living Skills Assessment or the NDIS Home and Living Supporting Evidence Form. OTA can also provide formal letters of recommendation for specific support needs, equipment, or urgent housing requirements.

After you have attended your virtual appointment with us, we will get to work on your required reports.
We guarantee that you will receive your completed reports within 2 weeks of your virtual assessment.
2. Sign Services Agreement
We will send you a digital services agreement to sign electronically.
Once signed, we will work with you to book a virtual appointment time that suits you.
